Neurokin is a small specialty practice. We're not trying to be available everywhere. The states where we practice are the states where Karina holds active clinical licensure — five states, with different scope in each based on local regulations.

New Jersey

Adult ADHD
Comprehensive three-visit evaluation, integrated diagnosis, written care plan, and ongoing care including medication management. $1,650 evaluation across three visits.
Adult Autism
Comprehensive three-visit evaluation (Visit 1 extended for adult autism), integrated diagnosis, written care plan, and ongoing care including medication management for co-occurring conditions. $2,050 evaluation across three visits.
AuDHD (autism + ADHD)
Comprehensive three-visit integrated evaluation, integrated diagnosis covering both neurotypes, written care plan, and ongoing care including medication management. $2,500 evaluation across three visits.
Chronic Migraines
Migraine evaluation and treatment, including Botox for chronic migraine when clinically appropriate. $425 initial consultation. Botox $2,150 per session, $8,000 annual bundle. Available at our Westfield NJ office.
How we work in New Jersey
In-person at our Westfield NJ office (560 Springfield Ave, Suite I) or telehealth statewide. Most patients use a mix — in-person for evaluations or Botox, telehealth for follow-ups.
Karina is DEA-licensed in New Jersey for full-scope prescribing — including controlled substances (ADHD stimulants) and any medications associated with co-occurring conditions or migraine treatment.
$175 each. Existing patients in continuous care since the original launch are grandfathered at the original $150 rate.

How we work in New York
In-person at our Midtown Manhattan office (1-2 days per week) or telehealth statewide. Most NY patients use telehealth for the majority of visits, with in-person for select evaluations or follow-ups.
Karina is DEA-licensed in New York for full-scope prescribing — including controlled substances (ADHD stimulants) and any medications associated with co-occurring conditions.
$175 each. Existing patients in continuous care since the original launch are grandfathered at the original $150 rate.
Migraine and Botox services are not currently available in New York. Migraine treatment is offered at our Westfield NJ office only (Phase 1).

How we work in Washington DC
Telehealth across DC. The full three-visit evaluation works well by video — most assessment work translates cleanly, and many patients find video evaluation easier than in-person clinical settings.
Karina is licensed in DC for evaluation and care planning, but doesn't prescribe in DC. Medication management happens through your local prescriber — most patients coordinate easily with their existing PCP, psychiatrist, or therapist using our care plan as the clinical guide.
Our care plan is detailed enough that most local providers can pick up directly from it. We can also offer perspective on what to look for if you're searching for a local prescriber.
Care plan updates: Follow-up visits are $175 each, by telehealth, if your care plan needs revision later (new circumstances, medication changes). Most patients do this once or twice a year.

Karina is licensed in Florida for evaluation and care planning, but doesn't prescribe in Florida. Medication management happens through your local prescriber — most patients coordinate easily with their existing PCP or psychiatrist using our care plan as the clinical guide.
Our care plan is detailed enough that most local providers can pick up directly from it. We can also offer perspective on what to look for if you're searching.

Telehealth statewide — Bay Area, Los Angeles, San Diego, Sacramento, Central Coast, Inland Empire, anywhere in California.
Karina is licensed in California for evaluation and care planning, but doesn't prescribe in California. Medication management happens through your local prescriber — most patients coordinate easily with their existing PCP or psychiatrist using our care plan as the clinical guide. The Bay Area and LA both have strong neurodivergent-aware provider networks.
Our care plan is detailed enough that most local providers can pick up directly from it. We can also offer perspective on what to look for if you're searching for a local prescriber.

I Don't See My State
Neurokin currently practices in five states: New Jersey, New York, Washington DC, Florida, and California. We add new states selectively — based on demand and Karina's licensure capacity, not on growth pressure.

If you're seeking adult ADHD evaluation
Look for ADHD-CCSP credentialed providers in your area, or ask local providers about their experience with high-masking adult ADHD presentations. Insurance-based ADHD evaluation is widely available; specialty private-pay evaluation matching our depth is rarer.
If you're seeking adult autism evaluation
Search for providers specializing in adult-onset evaluation rather than child/adolescent practices. The Embrace Autism online directory and the AANE provider lists are starting points. Look for providers using validated adult tools (RAADS-R, AQ-50, CAT-Q) rather than only childhood-focused criteria.
If you're seeking AuDHD evaluation
This is rare nationwide. Most providers do ADHD or autism, not both integrated. We are happy to suggest what to look for: a provider who explicitly assesses for both neurotypes in the same evaluation, uses tools designed for each, and produces an integrated care plan rather than two separate ones.

Your Questions About Where We Practice, Answered
Why these five states specifically?
Five states represent the active clinical licensure Karina holds. New Jersey is our home state and where the physical office is. New York is where Karina holds full DEA licensure for prescribing. Washington DC, Florida, and California are states where Karina holds clinical licensure for telehealth-based evaluation and care planning. We add new states selectively, not based on growth pressure.
Why isn't prescribing available in DC, FL, and CA?
Karina holds DEA registration in New Jersey and New York. The DEA registration governs the ability to prescribe controlled substances (including ADHD stimulants). DEA registration is state-specific, and maintaining it across multiple states involves ongoing administrative requirements that we focus on the states where we provide ongoing care directly. In DC, FL, and CA, your local prescriber takes the prescribing role using our care plan.
Is the evaluation different in different states?
No. The evaluation work is identical across all five states. Same three visits, same total clinical time (3 hours for ADHD, 4 hours for Autism, 5 hours for AuDHD), same comprehensive assessment, same diagnostic depth, same written care plan. The price is the same too. All evaluation tools (Creyos, QbCheck, validated rating scales) are included regardless of state. The only thing that varies is who manages medication afterward — Karina (in NJ/NY) or your local prescriber (in DC, FL, CA).
Can I do the evaluation by telehealth in NJ or NY?
Yes. Telehealth is available in all five states, including NJ and NY. Many NJ and NY patients prefer telehealth for the convenience. In-person is available at our Westfield NJ office and Midtown Manhattan office for patients who prefer face-to-face evaluation.
What if I move from one of your states to another?
If you move from DC, FL, or CA to NJ or NY, Karina can take over your medication management directly. If you move from NJ/NY to DC, FL, or CA, we transition prescribing to a local provider in your new state using the same care plan that's been guiding your care. If you move to a state we don't practice in, we help you find a clinician where you're going.
Do you plan to add more states?
Possibly. We add states selectively based on patient demand and Karina's licensure capacity. We don't have a growth-driven roadmap. If you're in a state we don't serve and want to know if we expand, you can submit your email above and we'll notify you specifically if your state becomes available.
What happens if I'm visiting a state where you don't practice?
Your visit elsewhere doesn't affect your care if your home state is one of our licensed states. Telehealth follow-ups still work as long as you're physically located in your licensed state during the appointment. If you're traveling for an extended period, let us know and we'll figure out scheduling.
Do you accept patients from outside the US?
No. Karina's licensure is US-state-based. Patients must be physically located in one of our five licensed states (NJ, NY, DC, FL, CA) at the time of any clinical appointment.

Can I work with you if I'm in one state but my prescriber is in another?
Maybe. If you're in NJ or NY and we're providing care, prescribing happens through Karina directly. If you're in DC, FL, or CA, your local prescriber generally needs to be in the same state where you receive your medical care. If you have a multi-state situation, contact us and we'll figure out what's possible.
