QbCheck Objective ADHD Testing

FDA-cleared computerized testing platform we use in adult ADHD evaluation when objective measurement of attention, impulse control, and motor activity adds clinical value.

QbCheck is a 20-minute computerized continuous performance test with motion tracking. The patient watches a screen and responds to specific visual stimuli while the system measures attention, impulse control, and motor activity simultaneously. The result is objective data on the three core ADHD symptom domains.

QbCheck is FDA-cleared for adult ADHD assessment. We use it when objective testing would clarify the clinical picture — particularly in cases with subtle symptoms, diagnostic uncertainty, or when patients want objective documentation alongside clinical assessment.

What QbCheck Measures

Three core ADHD symptom domains, measured objectively over a 20-minute task.

Attention

Sustained attention over the 20-minute task. The system measures how consistently you respond to target stimuli, how often you miss targets, and how response patterns change over time as fatigue or attention drift sets in. Inattention is one of the core ADHD symptom domains, and QbCheck provides objective measurement against population norms.

Impulse Control

How often you respond to non-target stimuli (commission errors). Impulsivity is another core ADHD symptom domain — QbCheck measures the rate at which you respond when you shouldn't, which is one of the more reliable behavioral indicators of ADHD-related impulsivity.

Motor Activity (Hyperactivity)

Motion tracking via webcam captures fine motor movements during the test. The system measures total movement, frequency of fidgeting, and motion patterns. Hyperactivity in adults is often subtler than in children — adults learn to suppress overt hyperactivity, but the underlying motor restlessness often remains. QbCheck captures this in a way clinical observation alone usually doesn't.

How QbCheck Fits Into Our Evaluation

QbCheck provides objective measurement of the three ADHD symptom domains — but it's not a standalone diagnostic tool.

What QbCheck does well:

Provides objective behavioral measurement of attention, impulsivity, and motor activity. Generates results comparable to age-matched and gender-matched population norms. Useful for documenting ADHD-related cognitive and behavioral patterns objectively. Helps differentiate ADHD from anxiety-driven attention problems (anxiety typically affects attention differently than ADHD does, in patterns QbCheck can reveal).

What QbCheck doesn't do:

It doesn't capture the lived experience of having ADHD. It doesn't account for executive function challenges that don't show up in a 20-minute task. It doesn't differentiate ADHD from other conditions that can produce similar attention and impulse control patterns (severe sleep deprivation, untreated thyroid disease, certain medication effects). The clinical interview catches what QbCheck can't.

How we use it:

QbCheck generates one piece of evidence among several. We use the results alongside the clinical interview, developmental history, ASRS-v1.1 and Conners' rating scales, and any cognitive testing from Creyos. The integrated clinical picture is what produces the diagnostic formulation.

When We Use QbCheck

QbCheck isn't part of every ADHD evaluation. We use it when objective testing would add specific value.

When the clinical picture suggests ADHD but rating scales are borderline

Some patients have clear clinical history of ADHD but score in borderline ranges on standard rating scales — sometimes due to high masking, high intelligence compensating for symptoms, or anxiety obscuring the underlying pattern. QbCheck can clarify the picture.

When patients want objective documentation

Some patients explicitly want objective testing — for ADA accommodations, university disability services, professional licensing, or simply for their own confidence in the diagnosis. QbCheck provides documentation that's objective, FDA-cleared, and easy to defend in formal contexts.

When differentiating ADHD from other conditions

Anxiety, depression, sleep disorders, and other conditions can produce attention problems that look ADHD-like in a clinical interview. QbCheck data patterns are sometimes useful for differentiation — anxiety-driven attention problems often look different from ADHD attention problems on objective testing.

Practical Details About QbCheck Testing

  • How long does it take?

    About 20 minutes for the test itself, plus 5 minutes for setup. Total session is about 25 minutes.

  • Where do I take it?

    Either in our Westfield office or by telehealth using your own device. Both formats use the same FDA-cleared protocol; the platform validates the testing environment.

  • What's the setup?

    The platform requires a computer or tablet with a webcam (motion tracking) and stable internet. We send setup instructions before the visit. Most patients can self-administer with our remote support if needed.

  • Can I take ADHD medications before the test?

    If you're already on ADHD medication and we're testing to assess medication response, take your medication as usual. If we're testing for diagnostic purposes and you're not yet on medication, take the test in your unmedicated baseline state.

  • How are results delivered?

    Results are generated immediately after testing and uploaded to Karina's clinical record. Karina reviews them before your next visit and discusses interpretation in the context of everything else we've gathered. The integrated picture is what matters.

QbCheck Is Included in Your Evaluation Price

Many specialty practices charge separately for QbCheck testing — sometimes $200-$400 on top of the evaluation. We don't.

QbCheck is included in the price of your evaluation when we use it. ADHD evaluation $1,650. Autism evaluation $2,050 (when ADHD comorbidity assessment includes QbCheck). AuDHD evaluation $2,500 (where QbCheck is more frequently part of the assessment battery).

  • Is QbCheck FDA-cleared?

    Yes. QbCheck is FDA-cleared for adult ADHD assessment. The FDA clearance reflects validated evidence that the test measures what it claims to measure (attention, impulse control, motor activity) with adequate reliability and validity.

  • Do I need QbCheck to be diagnosed with ADHD?

    No. ADHD is a clinical diagnosis based on history and current symptom patterns. Many adults are accurately diagnosed without objective testing. QbCheck adds objective data when clinically useful, but it's not required for diagnosis. We use it selectively when it would add value.

  • Will QbCheck definitively prove I have ADHD?

    No. QbCheck generates objective data that informs the diagnosis, but it doesn't "prove" ADHD on its own. A positive QbCheck result is consistent with ADHD; it doesn't constitute the full diagnosis. Conversely, a negative QbCheck doesn't rule out ADHD — masking, compensation strategies, and acute factors can all affect test performance.

  • What if my QbCheck results don't show ADHD but I really feel like I have it?

    This happens occasionally. Some adults with ADHD perform within normal range on QbCheck due to compensation strategies, high motivation during the test, or acute factors that improve performance temporarily. The clinical interview is what drives diagnosis — QbCheck provides supporting data, not the final word.

  • How accurate is QbCheck?

    QbCheck has good psychometric properties — sensitivity around 86%, specificity around 84% in published validation studies. That means it correctly identifies most adults with ADHD and correctly excludes most adults without ADHD, but it's not perfect. Like any test, it works best as one piece of the overall clinical picture.

  • Can QbCheck differentiate ADHD from anxiety?

    Sometimes. Anxiety-driven attention problems often produce different QbCheck patterns than ADHD-driven attention problems — anxiety tends to affect attention more in stress-loaded situations, while ADHD attention deficits are more pervasive across contexts. The differentiation isn't perfect, but the data can support clinical differentiation alongside the interview.

  • Is QbCheck covered by insurance?

    QbCheck is included in your evaluation price at Neurokin — no separate testing fee. As far as insurance goes, we're private-pay across all services. Some out-of-network insurance plans reimburse a portion of evaluation costs after the deductible; we provide a superbill with appropriate codes.

  • Can I take QbCheck if I'm already on ADHD medication?

    Yes, but the interpretation differs. If you're already medicated and we're testing to assess medication response, take your medication as usual. If we're testing for diagnostic purposes (you're not yet on ADHD medication), test in your unmedicated baseline state. We discuss this during the consultation.
